Performance contract

I, , for the performance dated from agree to the following:

I agree to perform at the agreed upon date and time.

I agree to show up at least 30 minutes in advance of the performance to set up.

I understand that rehearsal and set up time is included in the total payment. No additional compensation will be made for rehearsal time for this event.

I understand that MAP Family Learning Center is a family-friendly venue, and my program and conduct will reflect this.

I agree to being photographed and videotaped, and I give permission to Remake Music LLC (DBA MAP Family Learning Center) to publish any content made from the performance for promotional purposes.

I agree to advertisements being included in the progam booklet.

In the event that I must cancel the event for any reason, I understand that Remake Music LLC (DBA MAP Family Learning Center) is under no obligation to make payment, and that any possible rescheduling and compensation must be renegotiated if possible.

My performance will be between 45 and 75 minutes in length.

Upon complete signature and counter signature by an authorized signatory, Remake Music LLC (DBA Map Family Learning Center) agrees, in return, to compensate you $600 for this performance, which includes all performers (in the case that you may have chosen to sub-contract performers for this performance), equipment, and any and all rehearsal time you may require to prepare for the event.
